Family Medical Group Northeast, participates with most insurance companies and bills those carriers directly as a courtesy for you as our patient.

It is your responsibility however to contact your insurance carrier to verify that we are contracted with your specific insurance company. Additionally, it is your responsibility to know what your benefits are prior to any services provided at our clinic. All services performed for you by our providers are your responsibility whether your insurance company covers these services or not.

At check-in it is your responsibility to assure that we have the correct insurance information on file. This includes providing us with your current insurance carrier card. If you do not provide an insurance card at time of service, you will be required to pay for all services per-formed that day at check-out.

Co-payments are required at the time of service. If you may have difficulty with payments, please call our billing department prior to your scheduled appointment.

As mentioned, we do bill the insurance carrier as a courtesy from the card you have provided to us. If no payment or communication has been received from this insurance carrier within 60 days from date of service, the services will become your responsibility. We will assist you in any way we can, but the services are your responsibility. Please understand that each insurance company has many different policies and plans, and though we wish we could tell you what your benefits are at time of service, it is impossible for us to know specific information about each insurance company and the benefits for all of their different policies.
